HPV Changes / Treatment

If you have been diagnosed with an HPV infection, I am here to provide you with thorough counseling. It is important to understand that not every HPV infection leads to precancerous changes, so immediate action is not always required. Together, we will discuss the best approach for you—whether that involves further investigation, monitoring, or targeted treatment.

If treatment becomes necessary, the use of trichloroacetic acid can be an effective option to address precancerous changes. I will support you throughout the entire process, ensuring you feel secure and well informed.

  • Trichloroacetic acid (TCA)

    Conization is currently considered the standard treatment for high-grade dysplasia and is highly effective with manageable risks. However, it involves removing part of the cervix, which can increase the risk of preterm birth in women of childbearing age. Although the procedure is minor, it is usually performed under general anesthesia, something many women may prefer to avoid.

    Under certain conditions, I can offer you a gentler alternative: treatment with trichloroacetic acid (TCA). Studies show that a single application of this method can achieve healing rates of up to 82%, and with a second treatment, the success rate exceeds 90%. However, it is important to note that trichloroacetic acid is currently used as an "off-label" therapy. This means that the substance does not yet have specific approval for the treatment of cervical dysplasia, but its use is possible based on positive study results and careful medical consideration.

    Whether this treatment option is suitable for you can be decided together after a thorough discussion of your findings and a colposcopy in my practice.

  • Condylomas (Genital Warts)
    Condylomas, also known as genital warts, are benign skin growths caused by the Human Papillomavirus (HPV). They primarily occur in the genital and anal areas and can vary greatly in size and number—ranging from small, inconspicuous bumps to larger, slightly raised skin growths. Condylomas do not always need immediate treatment, as they are often self-limiting and may disappear on their own. However, they can cause discomfort, such as itching or occasional bleeding. In such cases, various treatment options are available.
    Condylomas have a high recurrence rate, so combinations of different therapies are often a good option to prevent this. I would be happy to provide you with individualized advice on the best approach.

How often should I visit the gynecologist?

As a gynecologist, my main role is to provide counseling and preventive care. In terms of the Pap smear, an annual check-up is recommended. If there are concerns in the family history, such as breast cancer, ovarian cancer, recurrent cyst formations, endometriosis, or HPV infections, a semi-annual examination is advisable.

I have my period; can I still come for an examination?

If it's a routine check-up and the bleeding is not too heavy, you are welcome to attend your appointment during your menstruation. Generally, the ideal time for the examination is shortly after your period, as this is when the Pap smear can be optimally evaluated.

What happens during a routine check-up?

During a routine check-up, we always begin with an open conversation. How are you feeling? Is there anything that is troubling you? As part of the examination, we perform a Pap smear, a palpation of the internal organs, an ultrasound, and a breast examination. Starting at the age of 30, an HPV test is conducted regularly every 3-5 years. Additionally, I offer the option to perform tests for sexually transmitted infections, tailored to your individual needs.

What happens next if something unusual is found in a lab test?

If any abnormalities are found, I will personally call you. If only medication or an additional examination is needed, you can pick up the prescription or referral, or have it sent to you by mail or email. For more complicated findings, you will be promptly given an appointment for a consultation. If we are unable to reach you, you will receive an appropriate letter by mail.

What is the HPV vaccine?

The Gardasil 9 vaccine protects against nine types of HPV, including the two main causes of genital warts (HPV 6 and 11) as well as seven high-risk types (HPV 16, 18, 31, 33, 45, 52, and 58), which are associated with the development of cervical cancer and other cancers.

Since July 2024, the HPV vaccine is part of Austria's free vaccination program for individuals aged 9 to 30. Two doses are required for effective protection.

This free vaccination option is available until the end of 2025.

For individuals over the age of 30, the HPV vaccine is not free of charge, and three doses are administered.

How does treatment with trichloroacetic acid (TCA) work?

First, we will discuss your findings and address any questions you may have. Then, I will perform a colposcopy to determine if treatment with trichloroacetic acid (TCA) is suitable. If so, and you choose to proceed, the treatment can be done immediately under local anesthesia. Mild side effects such as brief lower abdominal pain or dizziness may occur, so I recommend not driving after the procedure.

In the weeks following the treatment, increased discharge may occur, lasting up to three weeks. To support healing, I recommend avoiding full baths, swimming, intercourse, and tampons. After about eight weeks, we will check the success of the treatment with a follow-up sample.

What treatment options are available for condylomas?

There are various approaches to treating condylomas, often tailored to the individual and the extent of symptoms:

  1. Conservative treatment (medications)
    Special creams or solutions can be applied to stimulate the immune system locally or help combat the condylomas. However, the effect is often visible only after several weeks.
  2. Surgical removal
    Condylomas can be surgically removed. The warts are excised under local anesthesia, which is particularly useful for larger or hard-to-reach condylomas.
  3. Cryotherapy (freezing)
    Cryotherapy involves the targeted use of cold to remove condylomas. This method causes the wart cells to be destroyed and shed due to the cold.
  4. Laser therapy
    Condylomas can be precisely and effectively removed using laser therapy. This method is especially suitable for stubborn warts and is often used when other treatment options have not been successful.
What is dysplasia?

Dysplasia refers to cellular changes on the cervix. In most cases, these changes are harmless and may resolve on their own. However, if they spread, these cells can develop into precancerous stages. Therefore, observation and possible treatment are essential.

What is a colposcopy?

A colposcopy is an examination of the cervix, vagina, and vulva using a special magnifying device called a colposcope. This allows me to better assess tissue changes that may be difficult to detect with the naked eye. The procedure is similar to a Pap smear and is usually painless or only slightly uncomfortable.

What are ovarian cysts and how are they diagnosed?

Cysts are fluid-filled cavities within tissue that are enclosed and separated from the surrounding area. They are often discovered incidentally during an ultrasound examination, as they typically cause little to no symptoms. Treatment is based on individual factors.

Why is oncological follow-up care with a gynecologist necessary?

After successfully overcoming cancer, oncological follow-up care primarily aims to monitor treatment side effects and potential long-term consequences, addressing them as needed. Additionally, it allows for the early detection of any recurrence of the disease, ensuring that it can be treated promptly.

Can I get an appointment on short notice?

For urgent issues, please contact us by phone. We will do our best to find an appointment for you as quickly as possible, depending on the nature of your symptoms.

How can I pay, and how is the reimbursement processed after an examination?

The fee can be paid in cash, by debit card, or credit card. Upon request, we will take care of the automatic submission of the private doctor's invoice directly at the office, ensuring a quick reimbursement. Currently, automatic submission is available for ÖGK and SVS.

Patients without insurance in Austria who use laboratory services will receive a separate invoice for the laboratory costs. These costs must be covered by the patient.
